About National POLST Collaborative
National POLST Collaborative (NPC) is an independent 501(c)3 non-profit organization that organizes the effort to standardize the POLST process, forms and education throughout the U.S., since POLST varies from state to state. We advance the use of POLST through a national vision of best practices around quality, shared decision-making conversations for those facing serious, advancing illness and frailty due to aging.
Our Mission
We engage stakeholders in the adoption of quality, shared decision-making conversation models for the seriously ill in all states and nationally.
We promote programs that honor treatment preferences and goals of care conversations, resulting in actionable, portable medical orders for the seriously ill.
Our Vision
Creating a world where all persons living with progressing serious illness and frailty due to aging have access to shared decision-making conversations regarding values and treatment preferences, reflected in a portable, standardized medical order set.

The Institute of Medicine, now the National Academy of Medicine, encouraged all states to create a POLST program that meets national standards in their 2014 report. National POLST Collaborative also has attracted the support of many other organizations:
Statement Against Racial Injustice
National POLST Collaborative condemns racism, both systemic and overt. This amounts to, among other injustices, a public health crisis. We join our state program leaders and national partners in supporting the protection of human rights, the reduction of health care disparities and improving access to person-centered, interdisciplinary care among all populations, with a focus on minority and underserved populations.
