Membership Options
We offer three membership types to match your role in this work:

State Program Membership – $1,000 annually
Designed for official POLST programs at the state or territory level. Includes: Discounted Annual Summit registration, discounted access to online training webinars for continuing education credit, consultation with National POLST Collaborative leaders, access to NPC library of past webinar recordings and resource library, voting privileges for Board membership and officers, use of NPC Member designation on program website and materials.

Organizational Membership – $1,500 annually
Ideal for health systems, academic institutions, national partners, or regional programs. Includes: Discounted Annual Summit registration rates, discounted access to online training webinars for continuing education credit, consultation with National POLST Collaborative leaders, access to NPC library of past webinar recordings and resource library, use of NPC Member designation on organization website and materials.

Individual Membership – $500 annually
For healthcare professionals, advocates, and champions of person-centered serious illness care. Includes: Access to member-only resources, discounted training opportunities, exclusive updates and advocacy alerts.
How to Join

Complete the membership application form.

NPC reviews your application (usually within 7 days).
3. Pay
Once approved, you’ll receive an invoice with payment instructions.
