National POLST Collaborative (NPC) brings expert guidance for POLST completion through a new collaboration with MyDirectives.
NPC is collaborating with MyDirectives, a provider of digital advance care planning (ACP) tools and secure, cloud-based storage for patients and healthcare access, to offer guidance for their online POLST completion/retention product.
This guidance will be available through their MyDirectives for Clinicians platform. If your state or organization uses or plans to use the MyDirectives for Clinicians platform, you can request to have the guidance tools implemented for no extra charge.
