In cooperation with the National POLST Collaborative, nearly all states are working to grow adoption of POLST as a way to help patients, families, and clinicians navigate serious progressive illness and/or frailty due to aging. Active members of the National POLST Collaborative
POLST programs are run by each state, not by National POLST. Our full directory of POLST programs includes updated information for every state and the District of Columbia. These programs are responsible for developing and implementing practices and legislation in their own states for patients at this stage of life. Professionals interested in getting more involved should start with their state contacts.
